PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health condition, the provision of health care to you, or the payment for health care services.

How We May Use and Disclose Your Health Information

For Treatment

We may use and disclose your PHI to provide, coordinate, or manage your health care and related services. For example, we may discuss your medications with your physician or contact other pharmacies to determine your prescription history.

For Payment

We may use and disclose your PHI to obtain payment for services we provide. This may include contacting your health insurance plan to confirm coverage or billing your insurance company.

For Health Care Operations

We may use and disclose your PHI for our health care operations, which include quality assessment, employee training, licensing, and accreditation activities.

Other Uses Without Authorization

Uses Requiring Your Written Authorization

We will not use or disclose your PHI for marketing, sale of PHI, or most uses of psychotherapy notes without your written authorization. You may revoke authorization at any time in writing.
